Requirements for Care Providers

- Completed 4 Hour Course For HIV
- RN or LPN license
- Level 2 Background screening on file with AHCA
- Valid CPR card from American
- Heart Association or American Red Cross
- Free of communicable disease statement dated within 6 months
- 2 hours of HIV/2 hour AIDS Certificate
- 30 hours of Infusion Therapy
- Certificate (LPN only)
- Professional Liability Insurance – recommended
- Current FL Driver’s License
- Current Automobile Insurance
- Current Vehicle Registration
Home Health Aides and Certified Nursing AssistantsPatrick Ting2022-10-27T10:17:07-04:00
- CNA License in Florida (or HHA certificate – 75 hours minimum)
- Level II Background Screening on file with AHCA
- Valid CPR Card through American Heart Association or American Red Cross
- Free of Communicable Diseases (dated within the last 6 months)
- HIV 2 Hour/AIDS 2 Hour Certificate – 4 Hour
- Alzheimer’s Training Certificate – 2 Hour
- Self-Administration of Medication Certificate – 4 hours (Must provide first 6 hours)
- Professional Liability Insurance – recommended
- Driver’s License
- Social Security Card
- Auto Insurance & Vehicle Registration – You must have your own transportation.
